Are you passionate about making a meaningful difference in the final stages of life? Exciting opportunity for an experienced Palliative Care Coordinator to join our Residential Care team full-time.

About the role

As the Palliative Care Coordinator, you will lead the development and delivery of a high-quality palliative care framework aligned with Carrington’s mission and values.

You will:

  • Provide clinical leadership and education to staff
  • Ensure compliance with aged care standards and best practice
  • Support residents and families with compassionate guidance
  • Drive continuous improvement through audits, quality initiatives, and evidence-based practice
  • Collaborate with internal and external stakeholders to enhance care delivery

About you – Our Criteria

You’ll bring a blend of clinical expertise, compassionate leadership, and a strong focus on delivering meaningful end-of-life care.

  1. Registered Nurse with tertiary qualifications and postgraduate studies in Palliative Care
  2. Minimum 5 years’ experience in aged care or healthcare, including experience in operational/leadership roles
  3. Demonstrated ability to lead, coach, and mentor clinical teams to deliver high-quality care.
  4. Comprehensive knowledge of palliative care principles, symptom management, and end-of-life planning
  5. In-depth understanding of aged care legislation, quality standards, and compliance requirements
  6. Excellent communication, interpersonal, and problem-solving skills
  7. Demonstrated ability to work effectively with multidisciplinary teams
  8. High level of empathy, compassion, and a person-centred approach to care
