An exciting opportunity to join and build our great team at Gratis Home Care has arrived for a registered nurse. Are you looking for a change and interested in working office hours from Monday to Friday? Do you wish to be part of an amazing team?

Our clients receive funding from the Government Home Care Package Program (soon to be Support at Home) and can access a range of services to support their independence including clinical services. You will report to our Team Leader – Clinical Community Care and will be required to visit multiple clients in different locations within their homes to undertake assessments and nursing visits. This is a permanent full-time position.

Key responsibilities

• Conducting clinical assessments including the initial assessment/onboarding of new clients.
• Developing and reviewing support plans to enable the delivery of quality client care based on clinical assessment.
• Delivering direct clinical care to clients such as wound care, diabetic management, medication administration and catheter changes.
• Providing advice to Care Managers in the ongoing clinical needs and management of clients.

About you

• Full, unrestricted, AHPRA registration.
• National Police Clearance
• General nursing experience, post registration
• A passion for caring for people and delivering a high standard of care.
• Excellent communication, customer service and interpersonal skills.
• Experience using administrative office systems - MS Office (Word, Excel, Outlook)
• A reliable vehicle, current Australian drivers license, car registration and comprehensive insurance. Please note, Gratis Home Care has work cars available for nursing staff to book and use for client visits.
• Effective communication and interpersonal skills including the ability to work and communicate effectively with consumers, their families, community agencies and other professionals.
• Ability to work in a fast-paced environment, working both independently and as a part of a team
