A rare and exciting opening exists for an experienced Registered Nurse to lead our dedicated team of Community Nursing professionals. The Position: Ongoing Part Time, 32.0 hours per fortnight

The Community Health Nursing Manager will provide effective leadership to the Community Health nursing teams to ensure a safe and high quality nursing service is provided to the community. To be successful in this role you will have a nursing background and a good knowledge of Community Health programs to enable you to lead the delivery of high quality and efficient community health and complex care nursing services. You will have excellent leadership and administrative skills to fulfil the full responsibilities of the role.

Your Team:

The Community Health Nursing services sit under the Community Wellbeing and Aged Care Directorate and have a focus on wellness and reablement, offering clinical support for people living in the community through an appointment based or home visit business model. The Community Health Nursing teams provide services under a range of funding programs including Diabetes Education, Continence Advisory, Women's & Adolescent Health and Breast Care Nurse.

Key skills you need:

• Registered Nurse with current AHPRA Registration and minimum 5 years nursing experience
• Experience or demonstrated capability to lead a clinical team of high-functioning healthcare professionals
• Understanding of community health nursing service delivery, models of care and Health Independence Programs (HIP), Commonwealth Home Support programs (CHSP), and Home and Community Care (HACC) Guidelines.
• Excellent communication skills and ability to motivate and connect with staff and other stakeholders
