Role Overview
As a Support Coordinator at One Culture Support Services, you will play a key role in assisting NDIS participants to understand and implement their NDIS plans. You will work closely with participants and their families to connect them with appropriate services, coordinate supports, and empower them to achieve their goals. This position requires excellent communication, organisational skills, and a commitment to person centred care.
Key Responsibilities
Support NDIS participants in understanding and implementing their NDIS plans.
Coordinate and connect participants with appropriate services and community resources.
Assist participants in building their capacity to manage their supports independently.
Regularly monitor and review participants' progress and adjust supports as needed.
Collaborate with families, service providers, and stakeholders to ensure effective service delivery.
Maintain accurate and up-to-date records and documentation in line with NDIS requirements.
Advocate for participants' rights and support them in achieving their personal goals.
Provide timely reporting and feedback to the organisation and external agencies as required.
Key Selection Criteria
Essential:
Demonstrated understanding of the NDIS framework and person centred care.
Experience in support coordination, case management, or disability services.
Strong interpersonal and communication skills to engage with diverse communities.
Ability to work autonomously while managing multiple tasks and priorities.
High level of cultural competency and commitment to inclusion.
Proficiency in maintaining accurate documentation and using digital systems.
Current Working with Children Check and NDIS Worker Screening Check.
Valid driver's license and access to a reliable vehicle.
Desirable:
Qualifications in Social Work, Community Services, Disability, or related fields.
Experience working with culturally and linguistically diverse (CALD) communities.
Understanding of trauma-informed and strength-based approaches.
