Care to Care is recruiting for an experienced Care Coordinator. The successful candidate will ideally have a strong background in Community Care. This position offers a potential pathway to Care Manager - leading the team of Care Coordinators in future.

About the Organisation:
Care to Care is a registered NDIS, approved HCP and DVA provider, delivering in-home assistance to Aged and Disabled clients. We enable our clients to live their lives independently in their own homes and local communities and pride ourselves on our personalised service delivery. We operate across Brisbane and our main office is located in Cannon Hill.
You will be part of a very supportive team, led by passionate and involved owners. From a clinical perspective, you will be part of a strong team of ENs, RNs and other professionals.
Main Responsibilities:
- Client liaison and onboarding
- Conduct Assessments, Care Plan development and Case Management
- Support Worker matching and scheduling of services
- Direct and indirect delivery of clinical care (within scope of practice), ie medication management, complex wound management
- Guidance and support to fellow Care Coordinators and Support Workers
- On-call, shared across the team, on a rotational roster
- Ongoing Continuous Improvement and Risk Management
- Compliance, Incidents and Feedback Management
- Administrative activities, using the Care Management System
Key requirements:
- Minimum of 3 years’ experience as Care Coordinator
- At least Certificate IV in Aged or Disability Care with assessment modules, preferably EN or RN with current AHPRA Registration
- Detailed understanding of different funding programs, in particular HCP - but also others (NDIS, DVA, CHSP etc.)
- Outstanding interpersonal and communication skills
- A good listener with a positive, warm and & friendly personality
- Must be a team player, supportive, personable and reliable
- Calm natured, high on emotional intelligence and mentally resilient
- A natural leader with experience in leading/transforming a team - make decisions/give directions and mentoring/coaching colleagues
- Excellent organisational and time management skills, with high attention to detail
- Professionalism, well presented, a good reputation in the market
- Respect for our staff, clients and their families
- Advanced computer skills
- Full drivers’ licence
- Registered and insured vehicle available for work purposes
- Smart Phone & data
- Current First Aid & CPR
- National Police Check, Blue card and NDIS Workers Screening Check or willing to obtain
We offer a competitive remuneration package, in line with qualifications and experience under the relevant award with an additional above-award premium. On-call is remunerated in addition and a performance bonus is on offer.
