Care Manager
The Care Manager will oversee day to day clinical operations with the support from the facility Manager. You will oversee clinical governance, admissions, care planning, AN-ACC assessments and support Registered Nurse and Care staff.

This position is based only 1.5 hours from Sunshine Coast and Hervey Bay.

Essential Requirements:

• Current Registration AHPRA registration essential
• IPC Lead qualification desirable or willingness to obtain
• Qualifications and minimum 5 years’ experience as a Registered Nurse.
• Experience in planning, implementing, and evaluating innovative, evidence based clinical systems and practices.
• Ability to plan and deliver a diverse range of work plan activities concurrently to a high standard.
• Excellent written and verbal communication skills
• Demonstrated leadership skills.
• Experience with admissions
• Excellent at developing, establishing, and sustaining interpersonal relationships.
• Genuine desire to participate in the provision of care to frail and aged people.
• Demonstrated Knowledge and understanding of the Aged Care Quality Standards.
• Knowledge and understanding of AN-ACC assessments will be highly regarded.
• Current or willing to obtain a NDIS Clearance Certificate
