About the role

This is a part-time or full-time contract position based in Hervey Bay, ending on the 30th of June 2026. Providing a supportive and flexible working environment, with no shift work or after-hours duties.

As a Clinical Care Coordinator, you will collaborate with the Clinical Lead and Operations Manager to provide essential services throughout the Country to Coast Queensland (CCQ) region. Your role will involve supporting clients with severe mental health conditions by delivering direct treatment and coordinating their care effectively.Bottom of Form

Responsibilities include:

  • Conducting thorough clinical assessments and developing tailored treatment plans
  • Delivering evidence-based psychosocial interventions
  • Managing a clinical caseload and conducting risk assessment with management plans
  • Engage in data collection and contribute to monitoring and evaluation
  • Collaborate with multidisciplinary teams and external stakeholders
  • Facilitate care planning and review processes
  • Monitor and escalate mental health needs as necessary

Qualifications, Experience and Skills

  • Current annual practicing certificate from AHPRA or registered with AASW
  • Tertiary qualification in psychology, social work, mental health nursing, or a relevant discipline
  • Experience with complex mental health
  • Experience working in cross-cultural settings, particularly with First Nations people
  • Experience working in trauma-informed and recovery-oriented service
  • Ability to work in a team within a diverse environment
  • Strong problem solving and analytical skills
  • Excellent organisational and time management skills
  • Excellent verbal and written communication skills
  • Proficiency with Microsoft Office and software

  • Police check
  • Blue card (not required if registered with AHPRA)
