Clinical Care Coordinator (6 months) - Hobart

Seeking an experienced Clinical Care Coordinator for a new role that has just become available with a NFP provider near Hobart. This is a 6 month contract role which has the potential to go permanent.

As the CCC you will assist in leading the clinical care of all residents and become an important member of the leadership team. You will work alongside an experienced Care Manager, Facility Manager and a senior management team to provide clinical support across the home.

About the role:

• Fully accredited until 2027
• Lots of support from head office - Quality, Compliance, ANACC, HR etc.
• Assist the Care Manager with the planning and coordination of care services within the facility
• Supervise and support nursing staff, ensuring adherence to quality standards and regulations
• Foster a sense of community and inclusivity within the home environment
• Participate in quality improvement initiatives and accreditation processes

Requirements:

• Current AHPRA registration - DIV 1
• Demonstrated experience as a CCC or CN within aged care
• Extensive clinical skills & knowledge
• Ability to lead and support a multi-disciplinary team
• Experience managing day to day clinical issues
