About Us?

The Institute for Urban Indigenous Health Ltd was established by its founding members to provide a coordinated/integrated approach to the planning, development and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the Southeast Qld Region.

The team consists of three program areas providing transitional support for clients within the inner city region (Inner City Referral Service), clients in their last 6 weeks of custody at 3 distinct correctional centre locations and supporting their transition from custody to the community (Prison Transition Service) as well as clients in the RBWH catchment area for aftercare following a suicide attempt or suicidal crisis (Way Back Support Service).

About the Role?

To The role will work under the lead of TSS Care Coordinators and TSS Team Leader to provide client supports in the following key activities:

· Facilitate effective support structures, including external agencies (i.e. liaising and referring with community organisations such as Micah Projects and other government agencies such as Centrelink and QLD Health), in order to meet the complexity of client needs;

· Support and empower client independence and interdependence through effective engagement, goal plan setting and tracking, and access to appropriate resources, opportunities and services within the community.

Day to Day Duties Include:

  • Assist in the coordination and delivery of services for clients identified with drug and alcohol and/or mental health problems, in accordance with a case management plan and/or team care arrangement
  • Provide culturally appropriate services to clients with identified mental health and/or drug and alcohol problems; which include; support and case management
  • Participate in the provision of effective quality case management practices, including;
  • Identifying and using client strengths to assist in the development of appropriate support networks within their community
  • Liaise and develop external relationships with external services/agencies to collaboratively address client needs, including working relationships with Micah Projects, Centrelink, and health services and link clients with these services where appropriate
  • Contribute to smooth transitions for clients who are accessing other services (such as counselling etc.), including clients who are transitioning out of a treatment service
  • Work collaboratively as a member of a multidisciplinary team, to provide integration and coordination between Aboriginal Community Controlled Health Services and other agencies, to facilitate the delivery of an efficient and responsive drug and alcohol and mental health service to clients.

About You?

  • Basic knowledge of the mental health and drug and alcohol service system and the range of treatment options available
  • Demonstrated ability to work as part of an inter-professional team, including knowledge and understanding of the roles and contributions of other professionals.
  • Demonstrated effective verbal and written skills with particular emphasis on interpersonal communication skills, confidentiality and record keeping.
  • Current C Class Drivers Licence (QLD)
  • Current Criminal History Check
  • Valid Blue Card

Desirable, but not mandatory:

  • Demonstrated experience working with Aboriginal and Torres Strait Islander people and /or communities
