360 Health + Community is looking to welcome Support Coordinator(s) to our metropolitan team!

360 Health + Community is an established, experienced and growing not-for-profit organisation delivering NDIS, Psychosocial Support, headspace (x3), suicide prevention, chronic health & aged care, allied health, Street Doctor, and specialised mental health services. We operate throughout the Perth metropolitan area, the Mid-West (out of Geraldton), and the Goldfields (out of Kalgoorlie). In delivering our NDIS support coordination and recovery coaching services we specialise in the psychosocial domain and work with complex participants, including those living with more than one disability.

Our NDIS participants include those with guardians, administrators, child protection and family services, in forensic or custodial settings, voluntary and involuntary treatment for mental health challenges, living in psychiatric hostels, supported accommodation or independently in the community. Our team work with participants and their whole stakeholder and family network across a wide range of challenges - NDIS plan goals and services, housing & accommodation, physical & mental health treatment & wellbeing, safety and risk, legal and financial, crises and life challenges.

About the role - Support Coordinators

The role is responsible for assisting and supporting NDIS participants with complex needs, primarily in the psychosocial domain, to build personal capacity to make decisions and choices about the implementation of all supports in their NDIS plan. The coordinators will work with participants to ensure the quality and appropriateness of their supports, and to assist in addressing life challenges that may arise (unwellness, changes in circumstances, safety and risks).

The roles include:

  • Reviewing participants' NDIS plans and ensuring appropriate supports and services are implemented.
  • Building capacity for participants who are experiencing multiple complex barriers and to assist them towards achieving their planned goals.
  • Coordinating supports for participants and liaising with NDIA planners, allied health and other community-based services.
  • Enhancing participants' capacity to coordinate their own supports.
  • Responding to enquiries, preparing information and reports as needed.

What we are looking for / being successful in the role, you will have:

  • A relevant tertiary qualification (psychology, social work, counselling) - highly regarded for Support Coordinator.
  • Experience as a support coordinator (support worker experience is not equivalent).
  • Experience in working with people with psychosocial and mental health challenges and ill-health (experience in the forensic and involuntary treatment domains highly regarded).
  • Detailed working knowledge of the NDIS.
  • Experience in the delivery of case management and coordination of services.
  • Ability to interpret a NDIS plan and recommend service options within the guidelines of the plan and budget.
  • Demonstrated skill in establishing empowering and supportive partnerships with participants with complex needs and their guardians, families and carers.
  • Sound verbal, written communication, and negotiation skills.
  • A current full drivers' licence (essential).

360 Health + Community offers:

An attractive remuneration package based on:

  • Support Coordinator $36.84 to $41.22 per hour PLUS superannuation. Starting rate based on the qualifications and experience you bring to the role.

The roles on offer are full-time (with an option for 0.8 FTE / 4 days per week by negotiation). We have on-going positions and fixed-term parental leave coverage positions available at this time.
