Hours: Full-Time (1FTE)
Location: Sonder Marion and other co-located sites around Metro Adelaide.

About the Role:
The Toward Home Alliance was formed in response to SAHA’s 2020 homelessness sector reforms. Led by Lutheran Care, it includes Aboriginal Community Services, Baptist Care SA, Mission Australia, Sonder, and The Salvation Army. With a vision to end homelessness, the alliance prioritises person-led, culturally appropriate, and integrated services, ensuring homelessness is brief and non-reoccurring. Every role plays a vital part in this mission!
The Toward Home Wellbeing Case Manager provides case management for individuals with complex health needs experiencing homelessness, conducts assertive outreach, and participates in the Toward Home Resolve outreach roster.
Experience Requirements:
- Demonstrated experience in engaging with homelessness or vulnerable individuals and communities
- Experience in working with Aboriginal communities and the ability to provide culturally appropriate support
- Demonstrated experience in working with people impacted by AOD and/or complex mental health.
- Have a good understanding of the local health system including referral pathways.
- A minimum 2 years’ experience in health/mental health case management.
Qualification and Other Requirements:
- Tertiary qualifications in Social Work, Social Sciences, Human Services or related field, or significant relevant experience.
- Current or willing to obtain a Working with Children's Check (WWCC)
