The Role:
To assist individuals and their families to identify their needs through assessment and individual planning, to develop meaningful goals and practical strategies to meet their goals in their home and community.
Create and carry out customised client-related support plan within predetermined periods. Work with complex participants with all age groups from 0-65 years. Plan, deliver and assess behaviour supports according to the needs of the individual in accordance with applicable legal and moral criteria. Plan, implement and review training and Behaviour support plans for participants, support staff, families and carers in positive behaviour support and specialist topics as required. Provide coaching, modelling, and support to families, support staff and managers to ensure the effective implementation of support strategies. Undertake comprehensive assessment and analysis of behaviour to inform development of behaviour support plans and strategies in accordance with assessment findings and the needs of the person with disability. Share knowledge and experience through collaboration and training to help staff use positive behaviour support strategies and minimise the use of Restricted Practices to ensure good work practice.
Essential Criteria:
- Must have Proficient or Advanced NDIS registration (core may be considered for other roles)
- A passion for supporting people with a disability and their families
- Current Working with Children Check, Police check and Current CPR training
- Ability to work independently and as part of a larger multidisciplinary team
- Exceptional time management skills
- Knowledge of the NDIS system and process (preferred but not essential)
