About the Role:
Intereach is seeking a dedicated individual with highly developed communication skills to join the Aged Care Services team in the role of Care Coordinator.
This position will be responsible for coordinating and facilitating in home services and supports to older people who are eligible via My Aged Care to receive in home and Community based supports within the Community Home Support Program.
As a Care Coordinator you will work as part of a team to promote independence and community participation for older people to help them remain in their own homes.
The successful applicant will have Community Services experience such as Aged Care Sector or relevant qualifications.
The successful applicant will have demonstrated experience in Person-centered care planning, experience with the Microsoft Office Suite, excellent time management and organisational skills and an understanding of the issues facing older people and their carers.

The position is a full time and can be located in any Intereach Office within Victoria.
This position has been classified under the Social, Community, Home Care and Disability Services Award as Level 4 with an annual starting salary of $85,126.08 + Superannuation + Salary Packaging.
