Your Role
Make a meaningful impact by supporting individuals on their recovery journey at Freeman House. In this role, you'll provide high-quality case management, group work, and support to clients in our AOD programs. improving health, wellbeing, and social outcomes for clients. As an AOD Case Worker, you'll deliver tailored support and interventions, with a key focus fostering recovery and empowering individuals to build brighter futures. You'll be part of a supportive, inclusive, and collaborative team that values continuous learning and professional growth.
Profile description:
Your Responsibility
Provide case management and treatment support to clients in line with best practices and the Society's AOD Model of Care.
Assist clients transitioning from withdrawal to residential rehabilitation and community-based services.
Facilitate group therapy sessions and provide individual psycho-social support.
Work closely with a multidisciplinary team to achieve positive client outcomes.
Maintain accurate client records and contribute to service improvement initiatives.
Engage with clients' families and significant others where appropriate.
Conduct psychosocial progress assessments and participate in case reviews.
You Will Need
Experience managing a case load and working with individuals with complex needs (AOD, homelessness, mental health, justice system involvement).
Diploma in AOD or a related community services field with substantial experience, or a degree in Social Work, Counselling, or other related services.
The ability to deliver evidence-based treatment approaches to support recovery.
Strong communication skills and the ability to work collaboratively within a team.
A commitment to ethical, professional, and culturally sensitive practice.
A valid NSW driver licence.
