- Band 6 $95,636 -$104,344 per annum (dependent on experience) plus superannuation.

About the role:
We are looking for someone with experience in the aged care sector and in managing Home Care Packages. AccessCare's team of Case Managers works with our clients and their carers to ensure that people in receipt of Commonwealth-funded Home Care Packages are supported to use their funds effectively and are assisted in remaining at home. A key part of your role will be to provide clients with information, support, and advice to help them make informed decisions about their care
What you’ll deliver:
- Management of client care that is in line with their assessed needs and the scope of the Home Care Package Program
- Effective management of Home Care Package budgets, supporting client to maximise use of their available funding
- Client assessments that consider all aspects of the person’s care needs and consider their preferences
- Comprehensive care planning, ensuring service provision is tailored to accommodate the client's needs, preferences and circumstances
- Effective teamwork - contributing to AccessCare's multi-disciplinary Case Management team and the broader organisation
What you’ll bring:
- A tertiary qualification in Aged Care, Allied Health or a related field.
- Proven experience in the management of Community services and/or Home Care Packages.
- High levels of organisational ability.
- Advanced ability to communicate with and support people from all all backgrounds and with complex care needs.
