About the role

The Settlement Case Management Coordinator is responsible for providing high-quality, needs-based case management support to newly arrived humanitarian entrants and eligible migrants. This role has a focus on complex case management, responding to complex presentations and referrals, providing advice and expertise to MiCare caseworkers, and ensuring effective service coordination and compliance with program guidelines. The Settlement Case Management Coordinator will develop and maintain relationships with key stakeholders, advocate for clients' needs, and contribute to continuous improvement activities to enhance case management service delivery and staff capacity. This position plays a crucial role in achieving positive client outcomes and ensuring the efficient and effective delivery of Settlement services.

  • Provide high-quality, needs-based case-management support, including outreach, advocacy, and low/medium/high-intensity case management, to newly arrived eligible migrants and humanitarian entrants.
  • Deliver short-term intensive complex case management as required.
  • Assist team leaders in developing and maintaining relevant casework staff capacity to deliver intensive case management under the Specialised Intensive Services program (Humanitarian Settlement Support Program) and medium intensity casework in Settlement Engagement Transition Support Program (SETS).
  • Offer guidance and expertise to the broader casework team to enhance complex case management practices and efficiently navigate relevant services/systems to facilitate positive client outcomes.
  • Provide promotion of and triage and intake assessment for (internal and external) complex case presentations and potential SIS referrals.
  • Develop and maintain relationships with key referral agencies and stakeholders to improve service coordination and delivery, referral pathways, and local collaboration. Advocate for clients' needs as relevant and promote MiCare and its services.
  • Maintain accurate and detailed client files and records, ensuring timely data entry and compliance with confidentiality and privacy requirements. Meet and monitor KPIs and Service Level Standards, taking corrective action when necessary.
  • Participate in networks, trainings, forums, Community of Practice (COP), and conferences to progress professional development.
  • Contribute to MiCare’s quality and continuous improvement activities, including incident and hazard reporting as required by the organization and funding bodies.
  • Implement legislative requirements, including mandatory reporting, child safety strategies, and the use of the MARAM Framework. Contribute to the Family Violence Information Sharing Scheme (FVISS) and Child Information Sharing Scheme (CISS), ensuring client confidentiality and privacy.

About you

  • At least 3 years' experience in case management in settlement context, or in relevant sectors that enhance/support for this role.
  • Complex case management experience highly desirable.
  • Working knowledge of policies and referral protocols relating to settlement services.
  • Specialist operating knowledge/experience in one or more service areas such as community health, family violence, housing and tenancy, or family/youth services.
  • Understanding of current Commonwealth and State policies related to immigration, settlement, income support, Child Safe Standards, MARAM Framework, and Information Sharing Schemes.
  • High-level negotiation, advocacy, communication, and relationship-maintaining skills.
  • Demonstrated application of Case Management Models (including complex case management) within the settlement context, employing a strength-based, client-centred, and trauma-informed approach that promotes self-efficacy and agency.

Qualifications

  • Tertiary qualifications in Social Work, Psychology, Community Services, or a related field.
  • Working with Children Check
  • Current Victorian Drivers licence
  • Willingness to undertake a police check and provide evidence of current influenza and full COVID-19 vaccinations
