About the role

A new and exciting opportunity awaits an experienced Community Care Services Manager to strategically and operationally lead our Victorian portfolio. As a vital member of Redeema’s Leadership Team, the role focuses on enhancing client experience and engagement, driving optimal client outcomes and ensuring prompt delivery of quality NDIS services and supports. We are looking for a savvy, well recognized and respected leader to manage across portfolios for frontline service delivery, SILs and manage a dedicated disability workforce. A leader who is self sufficient and confident with a strong desire to deliver unparalleled NDIS services to our valued Participants.

Key responsibilities

  • Provide leadership and support to the Community Care Service Team and frontline disability support workers.
  • Identifying, triaging, assessing, and on-boarding participants/ clients while ensuring a positive and seamless experience.
  • Manage and facilitate the interface and relationships with Hospital NDIS Leads, our Partners and relevant stakeholders to ensure excellence in service delivery and quality.
  • Provide leadership in delivering the outcomes for the SIL Portfolio ensuring optimum client outcome and delivery of Redeema's premium service model.
  • Build robust operational networks, systems and processes to manage the NDIS business and funding needs and develop the interface with IT, Finance, Marketing and HR.
  • Be an engaging and collaborative leader, driving a high-quality and supportive culture of valuing people, that is focused on outcomes through coaching and mentoring.
  • Network and liaise with external stakeholders, participants/families and care teams to ensure continuity of quality services, improved service outcomes and client satisfaction.
  • Develop human resource capability and ensure safe delivery of NDIS client services through implementation of care models and frameworks.
  • Utilize the appropriate government portals and pathways to report on restrictive practices, chemical restraints and behaviours or incidents within the expected timeframe.
  • Establish and maintain strong and sustainable relationships with funding bodies, statutory authorities and relevant stakeholders to ensure compliance and business continuity.

What you’ll bring

  • Experience in similar role(s) and management of NDIS frontline service delivery, Care Coordinators and Rostering Teams.
  • Management experience in developing and leading Supported Independent Living (SIL) with a minimum of 5 years experience in this portfolio.
  • A competitive edge through your established and trusted relationships with NDIS Hospital Leads and Rehab units across Metro Melbourne.
  • A sound commercial understanding of NDIS business across SIL, frontline operations and managing a dedicated disability workforce.
  • A strong and reliable network with a variety of SDA stakeholders across Victoria.
  • Tertiary qualification in health or disability or a tertiary qualification in business management.
  • You’ll bring an energetic, solution-focused, sustainable and growth mindset.
  • Credibility, reliability and be well networked to facilitate business and operational growth and development.
  • Having a strong leadership and management background with a minimum of 5 years experience in the NDIS sector.
  • Passionate about providing the best support to participants/clients, their circles of support, and Support Workers.
  • Understanding and appreciation of the community’s cultural diversity.
  • Demonstrated ability to work with a high level of independence and autonomy.
