About The Role

Our Care Team is the heart of our business and has a genuine passion and interest in enabling our consumers to remain living independently at home. We have an exciting opportunity for like-minded Care Partners, who will be responsible for managing a personal portfolio of clients.

As a Case Manager (Care Partner), you will play a vital role in managing Home Care Packages, ensuring our clients receive high-quality, person-centered care. This includes balancing the emotional and practical needs of care recipients, navigating complex conversations with tact and professionalism, and fostering long-term relationships built on trust. We are seeking multiple applicants with customer service or case management experience who will champion our consumer-directed care approach, work with care recipients to manage their care plan and budget, and provide ongoing support.

Job Description

  • Provide case management for clients, facilitating care plans and monitoring budgets.
  • Conduct regular check-ins with clients to review care goals and adjust plans as needed.
  • Address sensitive topics (such as changes in care needs or funding) in a compassionate, solutions-focused manner.
  • Escalate health or wellbeing concerns through established clinical pathways.
  • Manage administrative tasks, ensuring compliance with Aged Care Quality Standards.
  • Foster positive relationships with clients, their families, and service providers.

Skills and Qualifications

  • Strong emotional awareness and ability to handle difficult conversations calmly and professionally.
  • Genuine interest in working in the Aged Care sector and with Care recipients
  • Ability to work independently, make decisions confidently, and demonstrate problem-solving skills.
  • Experience managing client needs within the Home Care Package framework is advantageous.
  • Excellent communication and conflict resolution skills, with an emphasis on de-escalation.
  • Moderate proficiency in Microsoft Office 365 and other CRM systems, and My Aged Care portal.

Mandatory Requirements

  • Current federal police clearance (not more than 12 months old) and international police check if you have lived overseas in the last 10 years
  • Evidence of Australian work rights (passport, birth certificate, etc).
