Phoenix Specialised Youth and Disability Services P/L is a provider of Youth and Disability staff and NDIS support services to communities across Victoria. We have grown significantly and are looking to recruit a Level 3 Specialist Support Coordinator to work with the NDIS community, child protection, and forensic disability services.
The role requires a person that would be able to work well in a team environment and autonomously.

Key Responsibilities:
- Case Planning
- Supporting caseloads from 20 participants
- Assisting individuals and their families to understand and implement their NDIS plans
- Conducting care team meetings with external stakeholders and participants.
- Building individuals skills/capacity, empowering participants to exercise skills independently
- Monitor plan outcomes and expenditure to ensure the plan remains in budget.
- Build effective relationships with participants and their families while maintaining appropriate and professional boundaries.
- Ensure mainstream and community services meet their obligations to the participant (Housing, Education, Justice, Health)
- Excellent communication skills with experience in report writing and case notes. Clear and accurate record keeping of information, time, and billing.
- Ensure compliance and legislation is always adhered to and developing frameworks to support this.
Skills and experience:
- Demonstrated experience working with people with a disability.
- Previous experience in community services, disability employment services, mental health services or similar.
- Thorough understanding and commitment to NDIS Practice Standards, Work Health and Safety and Equal Opportunity.
- A problem solver who shows initiative and supports the delivery of best outcomes.
- Demonstrated experience and understanding of indigenous culture highly regarded.
- Extensive knowledge of local disability, mainstream and community services. Networking skills to increase knowledge of local services including funded and non-funded services.
- Experience in the Justice system desirable but not essential.
- Strongly demonstrated ability to plan, organise and manage one’s own workload.
- Excellent verbal and written communication skills.
- Experience in support planning, working within a person-centered Framework.
- Computer literacy skills for recording of notes, gathering evidence, writing reports, correspondence, and other documents.
- Knowledge of National Disability Insurance Scheme and relevant legislation including the NDIS Act 2013 and Disability Act 2006.
Qualifications
Bachelor’s degree in relevant field including but not limited to allied health, psychology and social work
And/or
Extensive Experience in NDIS and the Disability sector or similar.
Essential role requirements:
- A valid Driver's License
- Hold a current Working with Children Check for paid employment
- Clear National Criminal History Check obtained with in the last 6
- NDIS Worker Screening Check
- Evidence of your right to work.
- First Aid and CPR Certificate
- NDIS Orientation Certificate
- Evidence of COVID 19 + Booster vaccination or valid medical exemption
