Care Coordinator - Community Care

About the role:

We are currently looking for a person who shares our values and commitment to providing exceptional services to NDIS participants. The main focus of this role is to coordinate participant services and care through case management in accordance with the service agreement. The successful candidate will be required to provide leadership, problem solve, plan, liaise with external agencies and provide support and guidance to care staff.

Main Tasks:

• Liaise with all members of the care team and provide coordinated care
• Contribute to the development, implementation, monitoring and review of care plans to meet participant care needs and to promote quality of life
• Promote a consultative and collaborative approach to participant care planning and quality managed activities
• Carry out comprehensive, accurate and timely file reviews
• Maintain appropriate records and documentation in accordance with company policies and legislative requirements

To be eligible, you must:

• Have found knowledge of the industry with previous experience in a similar role desirable
• Be independent, self-motivated and able to manage your own diary with minimal supervision
• Be competent in the use of technology
• Hold the mandatory compliance required or be willing to obtain this prior to commencement
• Hold a qualification in community services
