About the opportunity

The Outreach Carer Services Navigator is a mobile role dedicated to raising awareness of Carer Gateway services and providing direct support to carers across the Lithgow/Nepean Blue Mountains, NSW region. Traveling in a Carer Gateway bus, the role involves engaging with local communities, carers, and health professionals in various settings, including carers' homes, community spaces, and healthcare facilities. A key focus is building partnerships with local agencies, identifying new carers, and acting as a direct referral point by collaborating with hospital discharge planners, social workers, and aged care assessment teams.

In addition to community engagement, the role involves consulting with carers to understand their needs, coordinating support plans, and facilitating peer support programs. Using principles of peer support, the CHIME framework (Connectedness, Hope, Identity, Meaning, and Empowerment), and reflective practice, the role provides carers with opportunities to connect, share experiences, and access relevant resources. This position requires adaptability, strong interpersonal skills, and a commitment to improving the well-being of carers in the region

To be successful in this role, you will need:

  • Lived experience as a young Carer or adult Carer
  • Certificate IV or above in a support discipline and/or Equivalent Community sector experience
  • Current valid Driver's Licence
  • Work in a mobile setting and ability to travel regularly
  • Satisfactory National Police Records Check (within 12 months)
  • WWCC / QLD Blue Card
  • NDIS Workers Screening check / QLD Yellow Card
  • Appropriate IT Skills
  • Right to Work within Australia
  • NDIS Worker Orientation Module Certificate
