TASSK is a NDIS provider, owned by a First Nations man with cerebral palsy. TASSK has a strong reputation in the industry, and therefore continues to rapidly grow. We are now looking for a dynamic Team Leader to guide our team through this growth phase.
As a NDIS Service Team Lead you will:
- Coordinate the intake of participants and their transition to services.
- Complete all client onboarding documentation and regularly review client plans, assessments and funding.
- Liaise with service providers, healthcare professionals, educators, and employers to ensure seamless service delivery
- Manage and coordinate a team of support workers and administration support staff including identifying training requirements and addressing performance issues.
- Create and manage rosters and care schedules in collaboration with the Rostering Team, adhering to award guidelines and NDIS pricing arrangements.
- Monitor, implement and complete reports as per NDIS Practice standards to meet compliance requirements.
- Engage in continuous quality improvement of systems and processes to ensure best practice, particularly in relation to the NDIS Practice Standards.
- Have availability to work within a rotating weekend on-call roster.
- Lead and guide into new areas of service (such as SIL)
We are looking for someone who has strong experience in:
- Building client and staff relationships to deliver quality person-centred supports.
- Rostering in accordance with the SCHADS Award.
- Report writing and maintaining participant records, including Service Agreements, care plans, risk management, complaints management, and incident management.
- Being a highly resourceful team player, comfortable working in a fast-paced environment, while remaining flexible, proactive, and efficient.
- An in-depth understanding of the NDIS Practice Standards, Rules, Code of Conduct, frameworks and guidelines.
- Leading a team beyond supervising - displaying real leadership skills to support, engage and inspire staff.
Additionally, you will need:
- Experience using PRODA, PACE and Nightingale.
- Working With Children Check.
- NDIS Worker Screening Clearance.
- Current valid Western Australian Driver’s Licence.
- Minimum 2 years experience
- An understanding of Supported Independent Living and other NDIS-funded living arrangements or willingness to learn.
