About the Role
As a Support Coordinator at DSP, you will play a crucial role in assisting NDIS participants in understanding, implementing, and maximizing the benefits of their NDIS plans. You will work closely with participants, their families, and service providers to coordinate and facilitate access to appropriate supports, ensuring that participants receive the highest standard of care.
The role requires a deep understanding of the NDIS framework, strong interpersonal skills, and the ability to work in a dynamic, participant-focused environment. You will be responsible for fostering meaningful relationships with participants while advocating for their needs and supporting them in building capacity for greater independence.
This role is ideal for an individual with a strong background in disability services, social work, mental health, or community services who is passionate about making a difference in the lives of people with disabilities.
Key Responsibilities
Participant Support & Coordination
- Assist participants in understanding and implementing their NDIS plans by providing guidance on available funding and services.
- Support participants in making informed decisions about their supports, promoting choice and control.
- Connect participants with appropriate service providers, community programs, and mainstream services.
- Assist participants in navigating complex systems, including health, housing, and employment services, to ensure they receive the necessary support.
- Develop individualized strategies to enhance participants’ independence, confidence, and ability to self-manage their plans.
- Regularly monitor participant progress and support them in adapting their plans as their needs and goals evolve.
- Coordinate case meetings, service reviews, and follow-ups to ensure service delivery aligns with participant goals.
- Work collaboratively with allied health professionals, disability support workers, families, and carers to optimize participant outcomes.
Capacity Building & Advocacy
- Empower participants by building their confidence and skills in managing their own supports over time.
- Advocate on behalf of participants to ensure their rights and needs are met within the NDIS framework.
- Provide guidance on self-management options, helping participants transition to greater independence.
- Assist participants in problem-solving challenges related to service access, funding utilization, and provider issues.
- Educate participants and their families on NDIS processes, policies, and available resources.
Administration & Compliance
- Maintain accurate and up-to-date case notes, progress reports, and service agreements in compliance with NDIS and DSP policies.
- Prepare reports and documentation to support plan reviews, funding applications, and service requests.
- Ensure all services are delivered in accordance with NDIS Quality and Safeguarding Standards.
- Stay informed about changes and updates in NDIS policies and industry best practices.
- Manage and coordinate billing and funding claims in line with NDIS financial processes.
About You
We are seeking an experienced and motivated professional with a passion for supporting individuals with disabilities. The ideal candidate will have:
Experience & Knowledge
- Previous experience in a Support Coordination, Case Management, or Community Services role within the NDIS or disability sector.
- Strong knowledge of the NDIS framework, funding categories, and service agreements.
- Understanding of the disability, mental health, and aged care sectors and their intersection with NDIS services.
- Experience working with diverse participant needs, including psychosocial disabilities, complex needs, and high-support requirements.
Skills & Attributes
- Excellent communication and interpersonal skills to engage with participants, families, and service providers.
- Strong problem-solving abilities and the capacity to advocate for participants effectively.
- Ability to manage a caseload while ensuring high-quality service delivery.
- Competence in using CRM and case management software for documentation and reporting.
- High level of organization and time management skills to meet deadlines and coordinate multiple services.
- Capacity to work independently and as part of a collaborative team.
- Strong conflict resolution and negotiation skills to address participant concerns and provider issues.
Qualifications & Requirements
- Diploma or Degree in Disability, Social Work, Mental Health, Community Services, Psychology, or a related field (preferred).
- NDIS Worker Screening Check & Working with Children Check (or willingness to obtain).
- Current Driver’s License and access to a reliable, insured vehicle.
- First Aid & CPR Certification (or willingness to obtain).
