Position Summary:
As a Support Coordinator, you will play a key role in empowering individuals with an NDIS Plan. Your responsibilities will include providing practical assistance and non-clinical care, helping clients organise and access their supports, and encouraging active participation in the community. By offering personalised, client-focused support, you will enable individuals to navigate the NDIS system effectively and access the services they need to thrive.

Key Responsibilities:
- Provide non-clinical care and practical assistance to individuals with an NDIS Plan.
- Support existing clients by offering personalized support tailored to their circumstances.
- Enhance clients' capacity to coordinate and access supports effectively.
- Actively engage clients in community activities and endeavors.
- Advocate for clients' needs within the NDIS system.
- Empower clients to navigate the NDIS system and access appropriate services.
- Spearhead efforts to expand Support Central Co's presence in the local community.
- Contribute to the growth and development of Support Central Co through impactful support services.
- Champion Support Central Co's mission of redefining industry standards and setting new benchmarks for quality care and services.
- Collaborate with colleagues to ensure seamless support delivery and client satisfaction.
- Stay informed about relevant regulations and guidelines related to the NDIS and disability support services.
- Continuously strive to make a real difference in the lives of clients and contribute to a brighter and more inclusive future
Selection Criteria:
- Previous experience in the NDIS Support Coordination role.
- Demonstrated skills in engaging with clients, family members, and service providers.
- Understanding of person-centered practice.
- Experience liaising with external service providers.
- Ability to network and develop effective working relationships.
- Proficiency in Microsoft Office programs.
- Demonstrated ability to adopt and champion change.
Special Conditions:
- Incumbents must undergo a current NDIS worker screening check.
- Must hold a current unrestricted NSW Driver's License.
- Community travel.
- Some out-of-hours work may be required, with flexible hours negotiable.
