About the Role: We are looking for an experienced Support Coordinator to join our dynamic team. In this role, you will assist NDIS clients in navigating their plans and accessing the services they need. You will work closely with clients, families and service providers to develop personalised support plans and ensure effective service delivery.

Key Responsibilities:

• Coordinate the implementation of NDIS plans.
• Develop and implement personalised support plans.
• Engage with participants, families, and service providers.
• Maintain accurate documentation and ensure compliance with NDIS requirements.
• Identify and manage risks and incidents.
• Drive continuous improvement through feedback and service reviews.
• Collaborate with the stakeholders and clients.

Skills & Experience:

• Proven experience in NDIS, Aged Care, Nursing, Disability Services, or a related field.
• Experience in support coordination and client support.
• Certificate IV or higher in community/disability/leadership.
• Strong communication and interpersonal skills.
• Commitment to client-centered care and continuous improvement.

Desirable:

• Experience with complex clients and/or SIL clients.
• Change management experience.
• Project management skills.
• Innovative problem-solving abilities.

Location: Northern Beaches and Western Sydney

Employment Type: Full-time

Salary: $80,000 - $90,000 plus superannuation
