The Role

We are seeking a dedicated and experienced Clinical Care Partner to join our growing team. This position combines clinical nursing expertise with care coordination and leadership to ensure our clients receive safe, effective, and person-centred care across Aged Care and Disability Support services.

You will work closely with participants, families, Support Workers, Allied Health Professionals, GPs and external stakeholders to provide holistic care and clinical oversight. This role also supports quality, governance and continuous improvement across our service network.

Key Responsibilities

Deliver and oversee clinical services including wound care, medication management, infection control and chronic disease management.

Provide clinical advice, mentoring and training to Support Workers and Care Partners.

Conduct comprehensive assessments, develop individualised care plans, and review outcomes in line with client goals, risks and preferences.

Coordinate and participate in multidisciplinary meetings and case conferences.

Support compliance with the Aged Care Quality Standards and NDIS Practice Standards.

Lead or contribute to incident reviews, clinical audits and quality improvement initiatives.

Foster strong, respectful relationships with clients, families and care teams to ensure consistent, compassionate care delivery.

Monitor health outcomes and escalate clinical concerns appropriately to GPs or specialists.

Maintain accurate, timely documentation in line with organisational and legislative requirements.

About You

Undergraduate Qualification in Human Services or Allied Health required.

Minimum 3 years’ experience in aged care and Home Care Packages.

Sound knowledge of clinical governance, risk management and quality frameworks.

Demonstrated ability to mentor, support, and educate care teams.

Strong clinical assessment and documentation skills.

Excellent interpersonal and communication skills with a compassionate, person-centred approach.

Confident using digital care management systems and technology.

Essential Requirements

Valid Driver’s Licence, NDIS Worker Screening, and Blue Card.

First Aid and CPR Certification.

Experience supporting people with complex health, dementia, or palliative care needs.

Understanding of trauma-informed and culturally safe care practices.

Familiarity with My Aged Care and NDIS systems.
