Position Title: Care Coordinator
Location: Hope Horizons Cancer Wellness Centre

Reports To: General Manager
The Care Coordinator plays a pivotal role in providing holistic, person-centred care to individuals impacted by cancer. As part of the multidisciplinary team at Hope Horizons Cancer Wellness Centre, the Care Coordinator supports clients across all stages of the cancer experience—from diagnosis through treatment, survivorship, and beyond.
This role blends clinical expertise with compassionate support, coordinating care, conducting assessments, delivering education, and promoting wellbeing through evidence-informed interventions aligned with the Hope Horizons model of care.
Key Responsibilities
1. Client Assessment and Care Planning
Conduct comprehensive biopsychosocial assessments for clients affected by cancer.
Collaborate with clients and their families to develop personalised care and wellness plans.
Identify unmet needs and facilitate appropriate referrals to medical, allied health and support services.
2. Care Coordination
Liaise with GPs, oncologists, and other health professionals to support continuity of care.
Act as a central point of contact for clients, helping to navigate the health system.
Monitor progress and adjust care plans based on client feedback and clinical indicators.
3. Clinical Support and Health Promotion
Provide nursing advice and symptom management related to cancer and its treatment, and/or short- and long-term side effects.
Organise and deliver evidence-based education on topics such as fatigue, nutrition, pain, and social and mental wellbeing.
Empower clients with self-management strategies and health literacy tools.
4. Team Collaboration and Program Delivery
Participate in multidisciplinary team meetings and case reviews as required.
Support delivery of wellness programs and group education sessions.
Contribute to the development and implementation of new services or initiatives.
5. Documentation and Evaluation
Maintain accurate, confidential, and timely documentation in accordance with organisational policy.
Collect outcome data to support program evaluation and continuous quality improvement.
Participate in audits, service reviews, and reporting requirements.
Selection Criteria
Essential
Registered Nurse with current AHPRA registration.
Minimum 3 years of clinical nursing experience (oncology, palliative care, or primary health preferred).
Demonstrated understanding of cancer care across the continuum and integrative cancer therapies.
Strong communication and interpersonal skills, with a compassionate and client-centred approach.
Proven ability to work independently and as part of a multidisciplinary team.
Commitment to holistic, evidence-based, and culturally safe care.
Desirable
Postgraduate qualification in oncology nursing, palliative care, or similar.
Experience working in a community health, wellness or not-for-profit setting.
Familiarity with distress screening tools and psychosocial assessment.
Knowledge of local cancer support networks and referral pathways.
