Aged Care Program Officer
24 hours per week | 2 Year Contract
Do you love a chat, solving problems, and making a real difference? We’re on the hunt for our next Aged Care Program Officer to be the connector who makes things happen!
Please note this role is a part-time (24 hours per week), and is contracted 2 years from commencement.

We support over 1.6 million people across our region, with a focus on aged care, mental health, Aboriginal and Torres Strait Islander health, digital health, and workforce development.
What You’ll Do
You’ll support strong partnerships and practical improvements between aged care homes, pharmacists, and general practice. Your day-to-day might look like:
Build Connections: Pair up aged care homes with local pharmacists and nurture trusted partnerships.
Keep Projects Moving: Facilitate smooth communication and collaboration across the system.
Grow Local Networks: Organise forums, training, and communities of practice to spark shared learning.
Share What Works: Distribute practical resources, updates, and tools across the region.
Champion Digital Health: Promote use of digital tools and provide education to support uptake.
Bring Programs to Life: Help implement SEMPHN initiatives on the ground - efficiently and impactfully.
Support Improvement: Contribute to reporting and evaluation so we can learn, adapt, and improve.
About You
You’re a people person, easily connecting with everyone you meet.
You’re a great communicator, open to feedback, and love working with different perspectives.
Are equally comfortable flying solo or teaming up to get things done.
Keep an eye on the details - nothing gets past you.
Thrive on solving problems and stay cool when plans change.
Are tech-confident and quick to learn new digital tools
Enjoy planning ahead and bringing ideas to life
Have experience in program or project coordination
Bonus points if you’ve worked in pharmacy, aged care or health - but it’s not a dealbreaker!
Have a valid driver’s licence and don’t mind hitting the road to connect with our region
