• Pay rate – $46.35 per hour + superannuation + up to $18,550 tax free + 99cents per km

We are looking for passionate and caring individuals to make a difference in our clients lives by proactively partnering with them to manage care plans to enable continued independence and a high quality of life in their community. You will take ownership of a caseload and have experience with Homecare packages within Aged Care. This role is mainly office based but you may need to travel to see clients across Northern Sydney as required.

About the Role:

Provision of quality personalised care to our valued aged care clients in their own homes through services such as:

• Ensuring quality care planning and budget management is provided to all clients as per the Aged Care Quality Standards and program guidelines by undertaking person-centred care planning
• Working effectively with internal and external stakeholders to deliver services that meet client changing needs
• Working in a safe manner, closely following all policies and procedures and ensuring compliance with quality standards

To be successful in this role you’ll need

• Genuine passion for providing high quality care to older Australians
• Drivers license and a reliable vehicle
• Experience in Case Management including budgets
• Strong communication skills and a positive attitude
• Sound computer skills especially Microsoft Office products and ability to pick up new systems
• Willingness to apply for a Police Check
