About the Role
We are seeking a compassionate and experienced Community Care Registered Nurse to join our supportive and dedicated team. In this rewarding role, you will deliver high-quality, person-centred care to clients in their homes, promoting comfort, dignity, and independence while offering guidance and reassurance to families and carers during important stages of life.
Working collaboratively within a multidisciplinary team, you will provide expert clinical care across the client journey. This includes conducting comprehensive assessments, developing and implementing care plans, managing symptoms and pain, and delivering ongoing nursing support. Your clinical expertise and coordination skills will help ensure each client and their family receive the highest standard of care and support.
Strong communication is essential for success in this role. You must have proficient written and verbal English skills to ensure clear, accurate, and compassionate communication with clients, families, and team members.
This position operates on a rotating 7-day roster, with shifts available across days, evenings, and nights—offering variety, flexibility, autonomy, and the chance to make a meaningful impact every day
Key Responsibilities
· Provide holistic, evidence-based nursing care in the community setting.
· Undertake thorough assessments and develop individualised care plans.
· Deliver effective pain and symptom management strategies.
· Coordinate care in partnership with families, carers, GPs, and other community services.
· Support and educate families and caregivers, helping them feel confident and informed.
· Contribute to the ongoing development of palliative care services within the team.
What You'll Bring
· Current Registered Nurse (RN) qualification with AHPRA registration.
· Additional qualifications and/or strong experience in high physical care.
· Demonstrated experience in community-based nursing.
· Advanced assessment, care planning, and case coordination skills.
· Ability to work both independently and as part of a multidisciplinary team.
· Strong communication, interpersonal, and negotiation skills.
· A genuine passion for providing compassionate, person-centred care.
Essential criteria:
· The legal right to work in Australia.
· A valid driver's license and a reliable vehicle
· National Police Check
· Working with children check
· Certificate in First aid & CPR (current)
· NDIS Worker Screening Check
· Two professional references
· ABN Holders Only Apply.
