We’re seeking an experienced Clinical Case Manager Team Leader to provide leadership, guidance, and clinical oversight to our dedicated Clinical Case Management team. In this pivotal role, you’ll ensure the delivery of high-quality, person-centred services aligned with the Strengthened Aged Care Quality Standards and relevant program guidelines.
About the role
Reporting to the Programs Manager, you’ll oversee the coordination and oversight of key programs including:
- Support at Home and Restorative Care Pathways
 - Assistive Technology and Home Modifications (AT-HM) Scheme
 - End-of-Life Pathway and Supplementary Services
 
You’ll play a key role in:
- Leading and mentoring Clinical Case Managers to deliver safe, high-quality, person-centred care
 - Supporting workforce utilisation and achieving program KPIs
 - Building strong relationships across internal teams and external partners
 - Driving continuous improvement, clinical governance, and compliance with aged care standards
 
About you
You’re a confident and compassionate leader who brings:
- Experience in clinical case management or community aged care
 - Strong understanding of aged care quality standards and regulatory frameworks
 - Exceptional leadership and mentoring skills to build a strong workplace culture
 - A collaborative and solutions-focused approach
 - Relevant tertiary qualifications in Nursing, Allied Health, or a related discipline
