Coordinate with GPs, child safety, mental health, hospitals, and other services to ensure cohesive care outcomes for young people in your community.

This full-time role of Clinical Nurse Specialist (Youth Primary Healthcare) supports our mission by providing a range of primary health and education services to young people and their children in a clinic setting; aiming to improve access to health care information and services; provide advocacy and service navigation support; follow-up and empower young people to self-manage their own health and independently access health services.

You’ll be a great candidate for this role if you have:

• A Bachelor of Nursing and current AHPRA Registration.
• Demonstrated experience working with young people and understanding of the issues impacting young families experiencing or at risk of homelessness.
• Experience in a general practice or primary health care setting; with sound knowledge of the health services sector.
• Understanding of the principles and requirements of The Royal Australian College of General Practitioners [RACGP] Accreditation
• Comprehensive understanding of Child Protection regulations and the mandatory report role.
• Strong communication skills, including interpersonal skills and writing skills and the ability to advocate effectively on behalf of others.
• Strong organizational and planning skills and the demonstrated ability to work independently and prioritize and manage competing demands.

Given the work that we do there will be few non-negotiables. You’ll need to be able to have, or obtain prior to commencement:

• Working with Children Suitability Card [Blue Card].
• National Police Check clearance.
• Evidence of vaccination, immunity, or medical exemption for the following preventable diseases - Measles, Mumps, Rubella and Varicella [Chicken Pox]; Pertussis [Whooping Cough]; and Hepatitis B.
