About the Role

As Care Manager, you will oversee the delivery of high-quality, person-centred care for each consumer, guided by capabilities approach across admission, assessment, care planning, delivery, and evaluation.
This pivotal role reports to the General Manager Residential Care and works collaboratively with Care Coordinators, clinical governance, other LHG teams, and external health professionals. Together, we support residents to live their best life, their way, every single day.

About You
You are a compassionate and experienced leader who thrives in a collaborative, values-driven environment. With a strong commitment to person-centred care, you bring clinical expertise, emotional intelligence, and a proactive mindset to everything you do.
In this role, you will:
- Partner closely with the General Manager Residential Care and Care Coordinators to deliver high-quality, person-centred clinical and personal care.
- Ensure compliance with aged care legislation, standards, and funding requirements.
- Lead the admission, assessment, care planning, and delivery process to create safe, positive, and tailored experiences for each consumer.
- Foster strong relationships with consumers, families, care teams, and health professionals to support coordinated care and shared decision-making.
- Provide guidance and mentorship to Care Coordinators, Registered Nurses, and clinical staff, promoting consistency and excellence in care practices.
- Champion a culture of safety and continuous improvement, using data insights and regular audits to enhance service quality.
- Respond to feedback and complaints with empathy and accountability, ensuring consumer voices are heard and respected.
- Contribute to workforce planning, performance reviews, and staff development to build a skilled, compassionate, and compliant team.
- Apply a risk management approach to monitor and respond to high-impact and high-prevalence risks always respecting consumer choice.
The successful candidate requires a current National Police Clearance. In accordance with current health guidelines for the aged care sector, candidates are encouraged to have all recommended vaccinations.
