About the Role

You will:

Coordinate and manage services for Support At Home and NDIS participants

Develop, review, and monitor person-centred care and support plans

Conduct in-home assessments and identify risks and opportunities for improved wellbeing

Build positive relationships with clients, families, and support staff

Work collaboratively with the clinical team and other providers to ensure high-quality outcomes

Ensure compliance with Aged Care and NDIS Quality Standards

About You

You are an empathetic, organised, and proactive professional who enjoys supporting people to achieve their goals. You’ll bring:

Experience in case management, care coordination, or a related role within aged or disability services

Strong understanding of consumer-directed care, NDIS, and the new Support at Home program

Excellent communication, time management, and problem-solving skills

Ability to work independently and as part of a supportive multidisciplinary team

Current Driver’s Licence, NDIS Worker Screening (or willingness to obtain)

Relevant qualifications in Aged Care, Community Services, Nursing, Allied Health, or Case Management
