Case Manager - Disability & Aged Care

An exciting opportunity to join a respected community care provider delivering Disability and Aged Care services across South Brisbane.

You’ll support a small, balanced caseload of clients living with complex needs helping them achieve their goals and live independently in their own homes.
Ideal for an experienced Case Manager or Registered Nurse seeking
stability, flexibility and a genuinely supportive team environment.

What You’ll Do:

  • Deliver holistic case management from intake through to assessment, planning, implementation and review
  • Coordinate and oversee NDIS and Home Care Package clients, including complex and high-support participants
  • Work closely with Support Workers and Allied Health teams to ensure person-centred outcomes
  • Provide clinical or operational guidance to staff as needed (RN support available)
  • Build strong, trusted relationships with participants, families and community networks
  • Travel locally to visit clients across Brisbane South Region

What You’ll Bring:

  • Nursing qualification & AHPRA registration (preferred, not essential)
  • Minimum 2 years experience in a Case Management role
  • Strong knowledge of NDIS, SIL, DVA and/or Home Care Package frameworks
  • Experience supporting clients with disability, dementia or complex care needs
  • Excellent communication, coordination and problem-solving skills
  • A compassionate, proactive approach and genuine commitment to person-centred care
  • Current Driver’s Licence and reliable vehicle
