About the role

We are currently seeking an experienced Hospital Social Worker to join a team at a private hospital in the Sydney Northern Suburbs. This role focuses on discharge planning within an acute inpatient setting, requiring strong hospital and acute care experience. The position is part-time (2 days per week), with one day ideally being Friday. The role is available ASAP until March

Key responsibilities include:

• Conduct comprehensive assessments for patients with acute and chronic conditions in a hospital setting.
• Develop and implement effective discharge plans, rehabilitation, and maintenance programs.
• Collaborate closely with multidisciplinary teams to ensure smooth transitions of care.
• Maintain timely, accurate documentation and reporting in accordance with hospital and company standards.
• Provide counselling and support services to patients and their families during hospital stays and discharge processes.
• Facilitate patient and family education related to care and discharge plans.
• Communicate effectively with healthcare professionals, patients, and families to support care coordination.

To be successful in the role you will have:

• Bachelor of Social Work (or equivalent)
• Eligibility/ Membership with the Australian Association of Social Workers (AASW)
• Demonstrated experience in acute inpatient hospital social work, particularly in discharge planning
• Familiarity with hospital systems and processes
• Valid Australian working rights and drivers' licence
• National Police Clearance, Working with Children Check, and Aged Care Check
