Senior Patient Care CoOrdinator - Ensuring Patient Care and Good Clinical Outcomes

About the Role:

We are seeking an open minded Care Coordinator with a background in mental health, counselling, and/ or drug and alcohol support to join our caring, multidisciplinary team.

In this phone based role, you’ll be the central contact for patients who are integrating into medical treatment at the clinic. You will also be working alongside the medical director and nursing team to ensure patients are getting the best from their medical treatments, guiding them through their care.

Key Responsibilities:

Phone Call appointments with patients checking on their introduction to treatment.

Accurate note taking and educating the patients on how to get the best results out of their treatment

Managing patients on mental health care plans and ensuring accurate reports of the patient's well being to medical teams.

Weekly Meetings with the medical and Nurse team reporting on patients outcomes

Phone, Inbox and letter drafting duties associated with the role.

Other Office Administration Duties

Wages start at $35-45 per hour casual employment, Part Time position and above award hourly of the right committed candidate. MA000027- Health and Services Award.
