This is a subcontracting opportunity. We are seeking passionate and proactive Disability Support Workers to join our team and help improve the lives of individuals living with disability. This role involves delivering person centred support across a range of settings, including in-home and community-based environments, with a strong focus on capacity building and active engagement. The successful candidate will be responsible for supporting participants with activities of daily living, community access, and goal attainment, while maintaining high standards in documentation and progress reporting. You will be intuitive to each individual's unique needs and committed to empowering them to live more independently and with dignity.

The ideal candidate will:

Have experience or willing to work with complex clients

Be skilled in accurate and detailed progress note writing

Provide active support in daily living and community participation

Be intuitive and responsive to individual needs

Ensure that capacity building supports are implemented and documented effectively
