We are seeking suitable applications for a highly capable and values-driven Social Worker to join our dynamic team at Butler Community Mental Health.
The position is full time permanent at 76 hours per fortnight, but we would also consider part time or job share.
What is our Social Worker role?
The MHPHDS Community Adult Mental Health Stream provides evidence based, specialised mental health services. The service is committed to working alongside service providers, consumers and carer stakeholders, to promote optimal mental health for those referred to our service. Each community team provides case management to ensure holistic health care while practical and achievable care plans are developed collaboratively with the consumer, family, carer and other services to promote sustained recovery. The consumer is supported to feel optimistic for their recovery and foster a sense of hope and their capacity to change is encouraged.
The position is within the Intensive Community Outreach Team (ICOT) service, functions as the community based clinical rehabilitation services to those consumers with severe and persistent mental illness having one or more of the following: a) severely impaired executive functioning, b) disrupted personal relationships, c) poor engagement with either clinical or non-clinical services, d) non-compliant with treatment plans, e) likely to be on a Community Treatment Order under the Mental Health Act, f) prolonged or significant accumulated inpatient admissions in a year and/or multiple ED presentations, g) multiple complex unmet needs.
The ICOT functions differently from other mental health teams in that it provides assertive case management (increased frequency of contact), care coordination, and utilise different strategies to engage the consumer and carers to assist with recovery.
