RVP Facility Services Pty Ltd is a registered provider under the National Disability Insurance Scheme (NDIS), dedicated to delivering a wide range of high-quality support services for individuals and people with disabilities. Our services include in-home care, Supported Independent Living (SIL), respite and overnight care, social and community participation, events, art and creative programs, community activities, and emergency assistance. We are currently seeking a qualified Community Worker to join our dedicated team at our office located at 17 Pursue Way, Pakenham, VIC 3810.

The Role

We are seeking a qualified Community Worker to join our dedicated team. You will play a vital role in supporting NDIS participants, helping them achieve their goals, access services, and participate fully in their communities.

Key Responsibilities

Conduct client assessments: Interview clients to determine the nature and extent of their needs, including personal, social, emotional, financial, and health-related challenges.

Plan and implement care programs: Develop, implement, and review individualised support plans tailored to each NDIS participant’s goals and requirements.

Monitor progress: Track client progress, review support plans, report on outcomes, and identify any barriers or changes in circumstances.

Facilitate referrals: Connect clients with appropriate agencies and services such as healthcare, housing, employment, counselling, and other community resources, ensuring referrals are timely, accurate, and aligned with client needs.

Assess community resources: Evaluate local health, welfare, housing, employment, training, and other services to identify gaps, improve access, and connect clients with the most suitable opportunities.

Liaise with stakeholders: Work closely with community groups, welfare agencies, government bodies, and private organisations to coordinate support, promote awareness of services, and advocate for clients.

Support families and care provision: Assist families, and provide education and care for children and people with disabilities in various settings, including adult service units, group homes, and government-supported facilities.

Prepare funding and resource submissions: Draft applications for NDIS support, grants, and program funding, and report to government bodies and relevant agencies to ensure ongoing support for clients.

Organise outreach initiatives: Plan and conduct community outreach events and information sessions to raise awareness of available grants, support schemes, and services for NDIS participants.

Qualifications & Skills

Diploma or higher qualification in Community Services, Disability, Social Welfare, or related fields

Minimum of 1 year relevant experience supporting individuals with disabilities or in community services.

Understanding of NDIS processes, standards, and participant rights.

Strong interpersonal, communication, and advocacy skills.

Ability to work independently and collaboratively as part of a multidisciplinary team.

Current Driver’s Licence, First Aid & CPR certifications, and relevant Police/WWCC checks.

Experience in case management, care planning, or community development is highly desirable.
