About the Role
We are seeking a compassionate and organised Placement & Home Brokerage Officer to join the Statewide CO-ACT Access and Community Brokerage team within CALHN. In this vital role, you will support the smooth transition of patients from hospital along their aged care pathway. Your work will directly contribute to improved patient flow and care continuity across South Australian Local Health Networks.
You will play a key role in discharge planning by coordinating admission paperwork, liaising with aged care providers, and ensuring appropriate community services are arranged. With a focus on patient advocacy, you will work closely with families and carers to align care plans with individual preferences, all while delivering essential administrative and logistical support. This is a rewarding opportunity to make a meaningful difference in patient outcomes within a collaborative and supportive team environment.
About You
You are a highly capable and adaptable professional who thrives in deadline-driven environments. With strong interpersonal skills and a keen eye for detail, you bring efficiency, consistency, and initiative to every task, supporting seamless operations in a busy healthcare setting.
Able to work flexibly and be able to work with deadlines
Confident communicator with strong verbal and written skills, able to interact effectively across all levels and adapt to changing workplace demands.
Proficient in Microsoft Office and records management systems, with the ability to quickly learn and apply new technologies in the workplace.
