At Uniting Communities, we believe in building a compassionate society where everyone can thrive. Our Streetlink Program provides an integrated primary health and alcohol and other drug (AOD) treatment service for homeless and vulnerable young people.
We are seeking a Community Health Nurse (Clinical Nurse Level 2) 2 - 3 days per week to join our dedicated multidisciplinary team in Adelaide. In this role, you will deliver holistic, client-centred healthcare that supports individuals and their families to actively manage their health and wellbeing.
What you’ll do:
Deliver comprehensive clinical nursing services including immunisations, ante/post-natal care, child and adolescent health, AOD and mental health support, and sexual health services.
Provide outreach nursing services and health education to young people, families, and the wider community.
Advocate for clients, connect them with support services, and empower them to engage with community life.
Maintain accurate medical records and contribute to continuous service improvement.
What we’re looking for:
AHPRA-registered nurse with proven experience in community health nursing.
Strong clinical knowledge in areas such as adolescent health, mental health, sexual health, and AOD treatment.
Excellent communication skills and the ability to build trust with young people and families.
Commitment to Uniting Communities’ values: Kind, Bold, Genuine.
Current CPR, Child Safe Environments training, and unrestricted driver’s licence.
