About Us - Harold Hawthorne Community Centre
Harold Hawthorne Community Centre has been part of the Victoria Park community for over 60 years, supporting older people to live independently, feel connected, and enjoy life on their own terms. We’re now looking for a compassionate and motivated Care Partner to join our Home Support team.
This role is more than coordination – it’s about walking alongside clients as they navigate their care journey. You’ll be the trusted point of contact, tailoring and reviewing care plans that reflect each person’s unique goals, preferences, and circumstances.
You’ll be working with older people from diverse backgrounds and varying needs, so your ability to listen deeply, build trust, and advocate respectfully will make all the difference.
As a Care Partner, you’ll:
Carry out initial and ongoing assessments to understand client goals, health, and living situations.
Develop, monitor, and update care plans that are person-centred, practical, and compliant with aged care standards.
Coordinate and oversee the smooth delivery of home care services, ensuring clients receive the right support at the right time.
Support clients and families in understanding their funding, budgets, and care options.
Keep accurate, confidential records and ensure compliance with aged care legislation and the Centre policies.
Build strong partnerships with clients, carers, health and referral professionals, and service providers.
What you’ll bring to the role:
A minimum Certificate IV in Aged Care/Community Services (or equivalent).
Knowledge of the Commonwealth Home Support Programme (CHSP), and an understanding of Support at Home.
Experience in care coordination, case management, or aged care is desirable.
Knowledge of aged care standards, My Aged Care processes, and care planning.
Excellent communication, empathy, and advocacy skills.
Strong computer skills and a high attention to detail for documentation and reporting.
A collaborative, proactive, and positive approach.
Current WA driver’s licence and reliable vehicle to use for work purposes.
National Police Clearance (or willingness to obtain)
