We’re looking for a passionate and experienced care finder Senior Practitioner to provide specialist assistance to help people understand and access aged care supports and connect with other relevant supports in the community.

About the Role
As a care finder Senior Practitioner, you will be responsible for providing specialist and intensive assistance to help people within the care finder target population to understand and access aged care services.
You will:
Provide leadership, direction, and alignment for the iREACH care finder team with organisational goals and contracts.
Guide policies, data requirements, professional development, intake/triage, and team meetings.
Help foster a positive and effective workplace culture.
Lead and support care finders in client-focused activities, including:
Outreach and engagement with potential clients.
Building rapport and supporting access to My Aged Care.
Assisting with assessments, forms, and service navigation.
Encouraging service uptake and re-engagement if needs change.
Networking with providers and maintaining client follow-ups.
Assist the Centre Manager with service design, development, review, and reporting.
About You
You are an experienced, collaborative practitioner ready to lead. You bring:
A qualification or experience in Social Work, Nursing, Aged Care or Human Services
Detailed understanding of the aged care system and available services, including eligibility requirements
An appreciation of the challenges specific to older/ageing people and their carers
Experience in establishing and maintaining effective relationships with the community
Strong leadership skills and a passion for team development
High emotional intelligence, adaptability, and resilience
A commitment to culturally safe, inclusive, and trauma-informed practices
A valid Working with Children Check, or willingness to obtain one
