Disability/ Aged Care Support Workers – Dementia Experience | Community & SIL Support

As a passionate Disability Support Worker with experience in dementia care, you’ll be part of a vibrant team delivering high-quality, person-centred support through our SDA/SIL programs and community access services.

What we will offer You

· Mentorship and guidance from skilled leaders with up to 20 years of experience in the community services industry

· Comprehensive onboarding, including induction and a minimum of two buddy shifts

· Client matching to ensure alignment with your skills and experience

· Ongoing training to support your professional development

Main Duties & Responsibilities

· Collaborate on Care Planning
Work closely with participants, their families, and relevant stakeholders to develop and implement individualised Care Plans that reflect each person’s goals, preferences, and support needs.

· Personal Care Support
Assist participants with daily living activities such as showering, dressing, grooming, toileting, and eating, food preparation while promoting dignity and independence.

· High-Needs & Behavioural Support
Provide safe, respectful, and responsive support to individuals with high physical needs and/or behaviours of concern, following Positive Behaviour Support strategies and relevant care protocols.

· Communication Assistance
Support participants to use their preferred methods of communication, ensuring they are heard, understood, and responded to at all times.

· Relationship Building & Companionship
Foster positive, respectful relationships with participants by offering companionship, emotional support, and meaningful engagement in daily routines and community activities.

About You

You are passionate about making a meaningful difference in the lives of people with disabilities.

You are reliable, empathetic, and committed to delivering high-quality, person-centred care.

You bring experience or a strong interest in supporting individuals with dementia and complex needs.

You have a solid understanding of the NDIS, including SIL and SDA frameworks, and are confident working within these structures.

You are a strong communicator, a team player, and someone who values building genuine, respectful relationships with participants and their support networks.

Mandatory Requirements:

Certificate III in Individual Support, Disability, Mental Health

(Working towards or willing to obtain)

Industry experience in Australia, or personal lived experience

Valid driver’s license, Reliable Vehicle, Comprehensive Car Insurance

Current First Aid and CPR certificate

Paid Workers Blue Card (Working with Children Check)

NDIS Workers Screening Check (Yellow Card)

NDIS Modules (or willing to obtain)

Applicants must demonstrate consistent availability and a strong commitment to the principles of True Colours, ensuring their support practices reflect empathy, respect, and an understanding of individual differences.
