Peninsula Home Hospice (PHH) is an accredited not-for-profit organisation that provides specialist palliative care to people in the comfort of their home across the Mornington Peninsula, City of Frankston and part of the City of Kingston.

We are seeking a compassionate Specialist Palliative Care Nurse with a dedication to making a difference for client’s quality of life and to provide support to their families and carers.

Working within a multidisciplinary team the role provides specialist palliative care to clients and their caregivers along the palliative care trajectory. This includes assessment, ongoing care planning, including symptom and pain management, and nursing care.

You will use the outcomes of assessment to coordinate and plan appropriate ongoing nursing care in partnership with other services both within PHH and the community.

The role requires established work practices, a commitment to palliative care and the ability to work independently with guidance and support from a dependable nursing team.

This part time role will be a 3-4 day a week position across a 7-day roster.

What you'll bring

Your qualifications in nursing (RN1) and eligibility to work in Australia.

Postgraduate, or working towards, qualifications in palliative care

Ability to work within a multidisciplinary team with minimal supervision

Highly developed palliative care assessment and care planning skills

Experience working in a community setting

Excellent written and verbal communication/negotiation skills
