New NP Role – Transforming Aged Care Access | Work Remotely + Onsite Support

We’re excited to introduce a new collaborative care model supporting Residential Aged Care Homes (RACHs) — and we’re seeking a passionate Nurse Practitioner to help shape this initiative.

About the Role
In this contracted position, you’ll work closely with a dedicated GP to support aged care residents and staff by:

Triaging incoming calls from RACHs remotely

Conducting occasional onsite visits as clinically appropriate

Coordinating care between GPs, aged care staff, residents, and families

Facilitating short-term GP access and continuity of care

This innovative model ensures timely medical care for residents, enhances communication, and improves healthcare responsiveness across the aged care sector.
