The Role

An opportunity has become available for a Nurse Navigator (Child and Youth Mental Health Services) to join Gold Coast Health (GCH) where the focus of our 12 000-strong team is on providing world-class health care to people living in the Gold Coast, and northern New South Wales region of Australia.

You will

• Help young people and their families understand and access available services including National Disability Insurance Scheme, school supports, housing, and youth-specific programs
• Work closely with psychologists, social workers, psychiatrists, and youth workers to deliver integrated care.
• Collaborate with young people to co-create personalised wellbeing plans that reflect their goals, strengths, and support needs.
• Act as a central point of contact for young people with complex mental health needs.

About you

To be successful in this position you will require:

• A Bachelor of Nursing or equivalent
• Registration with the Nursing Midwifery Board of Australia
• Credentialed through the College of Mental Health Nurses (or be prepared to work towards a credential in Mental Health Nursing)
• A Masters or higher qualification within a relevant area is highly desirable
• Evidence of vaccination, or be able to be vaccinated against and remain vaccinated against - Measles, Mumps, Rubella, Varicella, Pertussis and Hepatitis B
• Tuberculosis screening, to identify if the tuberculosis vaccination may be required
• Declaration of serious discipline history must be disclosed
• Criminal History Screening will be required for new employees (GCH will cover cost)

Salary

• Permanent full time
• Base salary ranging from $132 562 - $143 924 per annum

About Us

The Child and Youth Mental Health Service (CYMHS) Nurse Navigator is an advanced practice nurse. The role provides a service response to the Gold Coast Child and Youth Mental Health Service working with young people and families 0-18 years old across the CYMHS catchment. The CYMHS services provide mental health assessments and treatment to young people and families who present with complex and severe mental health needs.

The Nurse Navigator will work in partnership with the consumer/parents/carer/family/supports, General Practitioners, specialists, allied health professionals, and non-government organisations involved in their care.
