About the Role
Based across the stunning Gippsland region - from Sale through to Lakes Entrance - you'll be part of a dynamic, multi-disciplinary Single Assessment Team, conducting home visits and assessments for older community clients. Using the Integrated Assessment Tool, you'll support clients to set meaningful goals, access the right services, and continue living safely and independently at home.
With the support of cutting-edge technology (laptop, phone and IT provided), you'll work 100% remotely, planning your own schedule and enjoying the variety of both in-home visits (predominately) and some phone-based assessments.
What You'll Do
Conduct comprehensive face-to-face and phone-based assessments via referrals from the My Aged Care Gateway
Develop Support Plans tailored to each client's needs and goals
Provide recommendations and referrals to aged care and non-aged care services
Travel across East Gippsland with a generous travel allowance provided (KPI: 2 assessments per day with travel time factored in)
What We're Looking For
Degree qualifications in Nursing, Allied Health or Social Work (essential)
Current AHPRA registration or eligibility for AASW membership
Ideally you will have had experience as an ACAS / RAS / Single Assessment Service Clinical Assessor OR 2+ years in community or residential aged care
Solid knowledge of MAC Portal, IAT tool, HCP/CHSP funding
Flexibility to travel across the region (Sale - Lakes Entrance)
Compliance with COVID-19 (3 doses) and 2025 flu vaccination requirements
Registered Nurses with AN-ACC experience in residential aged care are strongly encouraged to apply.
