About us:
Aged Care Assessment is a multidisciplinary team that assesses frail clients aged 65+ (50+ for Indigenous clients) to determine their care needs for residential, respite, community care, and rehabilitation programs, aiming to facilitate access to appropriate services through comprehensive, client-centered evaluations involving clients, carers, and providers across Central Queensland.
About the role:
The Aged Care Assessment (ACA) Senior provides leadership, training, and support to clinicians, conducting assessments in hospital or community settings per Commonwealth guidelines and the Aged Care Act 1997, with a focus on social work principles, comprehensive care planning, and ensuring quality, evidence-based practice through effective communication and collaboration within the public health system.
About you:
- Nurse Grade 7 - The successful applicant must hold a Nursing qualification and maintain the registration requirements with the (AHPRA) Australian Health Practitioner Regulation Agency, and Registration Board of Queensland including any necessary endorsements.
- HP4 - The successful applicant must hold a tertiary qualification and maintain the registration requirements with the (AHPRA) Australian Health Practitioner Regulation Agency.
- Previous Aged Care Assessment experience would be an advantage.
What we offer:
Total remuneration value of up to $164,212 p.a. is comprised of:
- An hourly rate between $66.85 - $72.58 p.h.
- 12.75% employer superannuation contribution
- 17.5% leave loading
